Job Overview

Key Responsibilities:

  • Conduct assessments to determine participants’ care needs, goals, and preferences, and contribute to developing individualized care plans in collaboration with the IDT.
  • Deliver and document Occupational Therapy interventions, including techniques, devices, and activities that support muscle re-education, cognitive function, balance, endurance, and other factors affecting Activities of Daily Living (ADLs).
  • Supervise and guide Therapy Assistants, ensuring effective implementation of Functional Maintenance Programs (FMP).
  • Provide training and instructions to staff, participants, and caregivers in occupational therapy techniques and practices.
  • Collaborate with the IDT to ensure seamless care transitions between settings (e.g., home, hospitals, skilled nursing facilities) and provide support in end-of-life care coordination.

Qualifications:

  • Relevant Tertiary Education equivalent to 4+ years in the field of study.
  • At least two (2) years of experience in a clinical setting, preferably with a frail or elderly population.
  • Registration with the National Board for Certification in Occupational Therapy.
  • Licensed or eligible for licensure as required in the state of practice.
